Make an Appointment

Please fill in the appointment request form below, selecting the physician/practitioner you wish to see as well as your preferred appointment day and time. Although we cannot gurantee that Raby Institute physicians and practitioners will be able to accomodate your preferrence we will make our best effort to do so. Patients of Dr. DuMont will have the option to schedule follow-up appointments either at the Raby Institute for Integrative Medicine or at Easter Seals in Villa Park, IL. You will receive a response from one of our staff members within 24 hours.
