How to Request an Appointment
To request an appointment, please fill in the appointment request form, selecting the physician/practitioner you wish to see as well as your preferred appointment day and time.
Although we cannot guarantee that Raby Institute physicians and practitioners will be able to accommodate your preference, we will make our best effort to do so.
You will receive a response from one of our staff members within 2 business days. If this appointment is urgent please call the office at 312.276.1212 to schedule. If this is an emergency, please dial 911.

Location & Parking
We are conveniently located in the heart of downtown Chicago’s Magnificent Mile at 500 N. Michigan Avenue, Suite 2100. Our offices are easily accessible by public transportation. If you choose to drive, we offer parking on Rush Street near the Nordstrom department store, 516 North Rush St.
Discounted rates are as follows:
4 Hours: $12
Customers will need to collect a validated parking ticket at the front desk of the Raby Institute for Integrative Medicine to receive the discount.
